Applying for Optional Coverage
Under most group insurance programs, a Plan Member is eligible to apply for optional life and optional accidental death & dismemberment insurance.
Optional Life Insurance
- Plan Member must complete an Application for Optional Group Life Insurance. To obtain this application, please contact your nearest Morneau Shepell office for more information.
- Take a photocopy of the application for your records and mail the original signed to Morneau Shepell for processing. Since optional life requires medical underwriting, the Plan Member and/or his/her spouse may be asked to provide additional medical information and/or testing by the insurance company.
Note: Optional life is NOT approved until the Plan Member is notified in writing from Morneau Shepell. You will receive a letter advising you of the insurerís decision (approval or declination) and the new coverage will be added to your billing statement by Morneau Shepell.
Optional Accidental Death & Dismemberment Insurance
- Plan Member must complete an Application for Optional AD&D Insurance. To obtain this application, please contact your nearest Morneau Shepell office for more information.
- Send a copy of the Application for Optional AD&D Insurance to Morneau Shepell and retain the original for your files. Since optional AD&D is not medically underwritten, coverage is automatic and the new coverage will be added to your billing statement by Morneau Shepell.
